What an auditor wants when a measurement goes out of spec

An out-of-spec reading is not the evidence. The response is. What an auditor wants to see after a measurement fails, and the record chain from the failed value to the action that closed it out.

Nobody who has run a press or a moulding cell believes a line of production stays inside tolerance forever. Tools wear, dies pick up heat, a shot count creeps and a dimension walks. A good auditor knows that too, which is why a stack of green readings does not impress anyone. What gets tested on the day is different. Show me a measurement that went out of spec, they say, and then show me what you did about it. A reading outside the specification with nothing recorded after it is the finding. Not the drift, not the wear, the silence.

The failed value is where the trail begins

A number outside tolerance proves one thing on its own, that on one part, on one characteristic, at one moment, the process was outside the specification. That is the opening of the story, not the close of it. What the auditor is really checking is whether the shop noticed, and whether the noticing turned into a chain of decisions that someone can walk end to end. The excursion is the trigger. Everything that answers for it is the evidence. If the trail stops at the failed value, there is no evidence, only a fault nobody owned.

Walk it the way an auditor walks it

Start with the measurement itself and make it specific. Which part, which characteristic, and which control plan revision was it captured against, because a reading judged against last year's revision answers a question nobody asked. From there the auditor follows containment. When that value came up out of tolerance, what happened to the material sitting behind it, the parts already run and the ones in the bins. Then disposition, the decision made on that contained stock, whether it was reworked, scrapped, used as is under a concession, or released after re-inspection. Then the harder step, root cause, which is not a restatement that the part was over tolerance but an account of why the process moved, a worn punch, a slipped setting, a fixture out of location. Then corrective action, what was changed, and the part that shops most often leave blank, whether it held when the process ran again. And last, who reviewed the exception and closed it, and on what date, so the loop has a name and a time on it rather than trailing off unfinished.

Keeping the chain joined in the tools you already work in

The reason this trail breaks is rarely bad intent. It is that the pieces scatter. The failed reading lives in one place, the containment note in a book, the disposition in an email, the root cause in someone's head, and by audit week nobody can lay them side by side. The fix is to make the response hang off the same record as the measurement rather than off to one side. When an out of spec capture result opens a tracked exception in Resolve instead of a note, the containment, disposition, root cause and corrective action are all recorded against the same part, characteristic and control plan revision the reading was taken against, and the capture results that come after show whether the action actually held.
